What do I do?

My mother recently suffered a debilitating stroke. Due to the stroke my mother is not capable of continuing to have a quality of life she considers acceptable. She is slowly declining medically but is very much cognitively there although the medical issues, and medications are slowly causing a decline in cognition. Although she and I have a wonderful support system I do find myself bearing the brunt of the decision making and support care. 

She is in Palliative care at the hospital awaiting transfer to a Hospice. I am struggling with what comes next. She has an apartment currently sitting empty. Bills are still getting paid, life is continuing on and I don't know what I should be doing in preparation for what is to come.

I am sorry for what you are going through.  If you simply need to have a place to "talk" to compassionate people - you have found it here.  Come back as often and as long as you need to.  We will be here for you.

It is so good that you have a wonderful support system.  Be sure to tell them explicitly what you need and what they can do.  Without this direction, people who often want to help, don't engage fully because they don't know how.

Speak with the hospice social worker for guidance as far as what comes next.  For specific logistics: these vary from person to person.  Perhaps the social worker can suggest local resources that can guide you.
