The Kelsey Trail Health Region's home care program helps people remain at home for as long as possible. It also prevents unnecessary hospital admissions and facilitates earlier discharge.
Services are provided on the basis of assessed need and are intended to help people who need acute, palliative and supportive care to remain independent at home.
Home care services include case management and assessment, nursing, personal care, home management, meals, respite and minor home maintenance. Volunteer services vary in each community but may include visiting, assistance with meals on wheels program and assistance with transportation to medical appointments.
