In each community of Fraser Health, there are program supports care for individuals with an advanced life threatening illness in a variety of settings: home, hospital, hospice or residential care. Services or resources may include: visits by a home care nurse; community health care workers to provide hands on care as necessary; consultation for managing symptoms such as pain and nausea; support for decision-making at the end of life; hospice volunteer visits; family conferences; access to acute care palliative care beds; equipment and supplies for at home care and grief and bereavement counselling. |
