The Mississauga Halton Community Care Access Centre (CCAC) provides in-home and community health care to residents of South Etobicoke, Mississauga, Oakville, Halton Hills, and Milton. Services include support for seniors so they can remain safely in their own homes and out of long-term care; palliative care to help people fulfill their wish to die at home with dignity; and rapid recovery programs for patients returning home following surgery. Counselling is avaiable to clients, families and caregivers throughout the transition to long-term care, as a last resort. |
